Membership Plan Terms & Conditions

*If periodontal disease or gingivitis is present, additional treatment will be necessary prior to your cleaning. Your dentist may also recommend a periodontal maintenance plan if necessary.

*Your Membership Plan and any discounts included do not cover treatments performed outside of Grand Avenue Dental Studio.

*Plans expire one year after purchase, and patients must complete services within the year to receive the Membership Plan’s included services and discounts.

*If paying with credit card or CareCredit, discounts and percentages of savings will decrease to 10% due to fees involved with financing programs and credit card processing.
